Job Summary

Do you have experience of case management and/or conducting assessments and the ability to chair and co-ordinate meetings?
We have an exciting opportunity for a Continuing Healthcare Assessor (CHC) to join our Funded Healthcare team. We are looking for someone who will align with our organisational values and will work with our team to support the screening and assessment of individuals applying for and already in receipt of CHC/FNC packages of care and be responsible for co-ordinating the most vulnerable people in society to ensure they are in receipt of the correct care.
Main duties of the job
If you are self-motivated and able to work under your own initiative, if you can work successfully both autonomously and as part of a team, if you have excellent time management and people management skills, and the ability to deal with difficult and challenging situations then this could be a great opportunity for you to join our team.
You will have the opportunity to liaise and work closely with the local authority, providers, patients and their families and other relevant agencies in assessing and supporting individuals, as well as developing care plans and review processes and making changes to care packages in accordance with the needs of the individual.

Job Description
Job responsibilities
You will work within a strong, friendly and supportive team, playing a key role in making recommendations on eligibility for CHC packages, as well as supporting the implementation of the national framework and national policies related to CHC/FNC.

The duties of the post holder will be based in and around the Kent region but they may be required to work in NHS SCW sites across the region the organisation operates in, in a flexible manner to maintain continuity of the service.
To facilitate flexible working, although this is the postholders primary role, they may be required to undertake work in another role or team, commensurate with the grade of this post, in order to utilise the postholders experience and skills and maximise the efficiency of the organisation
