Job summary
We are currently looking for a caring and motivated band 5 staff nurse to join our Outpatients Department team.
The successful candidate will be required to work within the Outpatient Department based at Northwich Infirmary.
Main duties of the job
- Organising and co-ordinating outpatient clinics, assessing care needs and developing, implementing and evaluating programmes of care.
- Working alongside other multidisciplinary teams.
- Provide specialist information and support within the outpatient setting to both patients and nursing support staff.

Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe and effective healthcare to the people of Cheshire and beyond. The Trust, which manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford, was established as an NHS Trust in April 1991 and became a Foundation Trust in April 2008.

We were also recognised nationally for our workforce health and wellbeing initiatives.

• Work collaboratively within the multidisciplinary team/agencies to ensure that patient needs are met.
• To be designated to manage the clinic for the duration of shift by clinic manager.
• Prepare the clinical area for safe practice by ensuring that cleanliness is maintained, materials and equipment are ready for use, including clearing away afterwards.
• To support in the management of a specific group of patients and delver evidenced based nursing care to the highest standard.
• Collect, collate and report information, maintaining accurate patient records/documentation.
• To respond to and prioritise a wide range of demands and tasks in a rapidly changing environment, liaising with other members of the multidisciplinary team.
• To contribute to the promotion of effective, appropriate and timely information and education to patients and relatives
